If you are covered by a prescription drug plan through your health insurance provider, your insurance provider is most likely working with an intermediary known as a pharmaceutical benefit manager (PBM).  The role of the PBM is to represent patients, but the PBM is often driven by the goal of company profitability, leaving patients with fewer brand options and higher out-of-pocket costs.  A few examples of drugs that are often affected include GLP-1 drugs, such as Ozempic® and Wegovy® for Type 2 diabetes, Humira®, and various insulin brands.
